Files left open after logoff.

What might be the cause of files being left open on the server after a user has logged off. Is it the application, network, server, client, NIC? The server is used only for one application and several users are leaving open files after they logoff (and power off). They are not closed until I delete the connection in Monitor.
 
In my experience, it has only been caused by 1-the application or 2-an improper shutdown of computer. What type of app? 16 or 32 bit? It is more common with older 16bit apps. If the app closes improperly or blue screen of death or dr. watson files can be left open. Your client interaction could have some bearingon it, what version of client?
