file upload suggestions

Hello,

I'm going to attempt something i've never tried before. what i need to do is have a button that someone could click to upload a file. i want the windows dialog box to open, where they can navigate to the file. then i need to upload the file to a directory on the network.

also, i need to be able to insert into the database the name of the file.

after the upload, i want to return to the initial screen.

any ideas, suggestions, or caveats?
