fiber optic ends troubleshooting

I wonder if this happened to others, our phone system connects locations with multimode fiber cable with ST connectors, and one of the locations went down one end was fine, and when checking the other end the ST connector "fell apart" in my hands, i.e. the white part of the st connector was stuck in the device, (I swear I was being gentle) Has this happened to others ? I have never had that happen before but it would certinally explain the "problem" as once the end was replaced and things where re-connected things where fine,
 
There are a lot of types and brands available out there that are built differently. With some, the ferrule (white part) snaps into the body after the fiber has been terminated. Others come as whole body. All depends on what was put in.
But, if it WAS a whole body connector, it would take something falling on it to do that. That's really what it sounds like anyway.
And it has happened to me before, generally on a Friday afternoon.
