Feature 986 drops callers on BCM 400 3.6 1

I have a customer with BCM 400 3.6. When they have a caller and they transfer to voicemail the caller gets disconnected. Any thoughts? I looked on Nortel's web page but didn't find anything on this issue.
 
Make sure autohold is enabled, under the capabilities of the set, on the set you're invoking F986 from. If autohold is disabled, the caller will get dropped.

Reboot shouldn't make any difference in this case unless the voicemail is locked up.
 
Re-boot did not work. Setting "Auto Hold" to Yes worked. I've never had to do that before on a system. It's always worked before. Is there a patch that is missing by any chance?
 
Not that I'm aware of. I don't think it spontaneously changes. More likely than not it got toggled off by someone in the programming.
 
Should also mention that autohold is enabled on sets by default.
 
All phones have two intercoms already.
