Favorites Icon disappears

I got this message from one of my clients. Do you know what could be causing this?

It seems that every 2 weeks our favicon and logo on the address bar stop
working. I cleared my cache and sure enough - neither of those came back.
Please advise on why we keep losing them and what we can do to have them
there permanently.

The code for the icon is this:

basically nothing, IE is notoriously flakey with favicons.

Hi mate,

If memory serves me correctly, you can right click on the bookmarked page and select change icon.

Now, have the favicon in a folder somewhere on your HDD but make sure it is not in the temporary internet files directory or any other directory that will be emptied.

As I said, this is only if I remember correctly, but might not work at all.
