Failed to reload from Safety backup

Having a MD-110 BC10/TA CNA 137 and made Safety backup to IO mega Jazz drive (2Gb disk), no problem making SAFETY-SF as SIPOS 0, but trying to reload, changing to SIPOS3 the LPU display shows:
3200
3210
5010
4122
5022
4122

Can see flashing light on disk when reading, but reload stops with 4122/5022 on LPU display.

Any tip?

What response do you get if you run "IODDP;"?
What result do you get if you run "FIFTI:SUBFS=SAFETY-SF;" ?
Are you sure that the SCSI bus connecting the drive to the NIU board is electrically 'terminated' correctly ?
