I am currently running 4.1 and need to disable external call forward on all phones. what do I need to change or add?

You could probably do that by configuring the Calling Search Space for Forwarded numbers to something that will not include routes that go to external numbers. If you want to restrict to internal numbers only, have all the DN's in an Internal partition, then have a CSS for CallFwd that only includes the internal partition. No partitions with routes out...

My suggestion is similar to loucanick. Choose "none" for the CSS for Call Forward All for the phones that you don't want them to have this ability. I don't know how to do it in several simple steps for all phones.
You may want to look into your default template for future phones set up as well.
In the old PBX date, this is by default not allow.
