Extension Lock Out

I have an SX200 ICP running 5.0.2.12 running two 5340e sub attendants. The second sub attendant was recently installed and is continuously locking out. I monitored the extension while they placed a call several times. Each time they disconnected the call, the status of the extension goes into suspended for several moments and eventually locks out. Why would cause an extension to behave this way?

Headset Enabled or bad hookswitch

Locked out occurs if the phone is off-hook too long (not hanging up)

Headset is not enabled. Wow, it's a brand new telephone. Equipment does go bad. Thanks!

Yep! The handset was plugged into the headset jack...
