Extension Billing

We have a client with a 403 who wants to setup Extn Billing using their LCR carrier - SkyBlue (UK).

The code is 1666, so I would expect to enter a shortcode as follows:

SC: ?
Tel No: 1666DE.
Feature: Dial

Or

Tel No: 1666,E.

Also tried

Tel No: 1666YE.

None of these codes work. However, if you create a code to get a secondary dial tone, and then dial 1666 wait one second and dial 201 then the phone number, it works.

Have tried several pauses in the shortcode too.

SkyBlue say they have Index customers who have it working, and I find IP Office can usually do anything Index can do.

Any ideas?
