I am going to use an SMDR 4 on a MICS. The PC with the TAPIT call accounting is located in another room. I would prefer to keep the SMDR unit in the phone room. I want to extend the SMDR output via Cat 5e cable and some DB-9 to RJ45 adapters.

I've tested it out, when i use the adapters, I get garbage. When I use a real serial cable, it work fine.

Any ideas on the proper wiring of these DB9-RJ45 adapters?

the first question is did you check out to make sure that the DB-9 -rj45 connectors are pinned corectly ie pin 1 from the male db-9 connects to pin 1 from the female db-9 etc.

There are db9-rj45 adapters that come pre-built (also DB25 to RJ45) Use these preferably. beyond that how long is your cable between equipment and can you get this to work if you take the computer into the equip room and use a stock Cat5 cable?

You can put the SMDR in the same room as the PC and use the existing phone's extension.
Why make it hard on yourself.
