Export data from tables in PDF to another program?

Hiya -
Is there a way that data entered into a pdf form can be exported into specific places in another program (specifically Excel and/or ACT?)

More specifically - I've made a pdf form with fields for names and addresses, etc. I want to know if I can populate an Excel database or an ACT database automatically with the data from the PDF fields.

I'm willing and ready to learn/teach myself whatever needs to be done to make this a possibility - if it's a possibility at all.
