Exclude Source not working on Opt 61C

We have a feature on our 2250 console switchboard called exclude source that allows the receptionist to announce a caller without the caller hearing what was said before connecting. On 2 separate occasions this month this feature did not work where the callers could hear the receptionist announce the calls. We have submitted 2 trouble tickets to our vendor and they just removed and then added the feature back which then will work. I was wondering has anyone had this problem and if so what was the resolution.
