What are peoples experiences of this please, especially on Exchange 2003?

I use the Commvault Exchange Agent to do an Exchange aware full backup and a couple of incrementals each day, and I've always tended to use Deleted Item Retention as the "get out of jail" card for recovering individual messages.

That said, we have a license, so I'm thinking I should use it, but I'm also thinking that the time it would take to do a message level backup of a couple hundred gig of stores and public folders would be horrendous?

It all depends on the size of your exchange servers. Our largest exchange server is about 750 gigs but the stores are maybe 80 at max. Since you can use Recovery Storage Groups in Exchange 2003 I do not do mailbox level backups since they take forever, and if the mailboxes are large I do mean forever. I can restore an 80 gig store to a RSG in about 40 minutes, mount the RSG store and then either merge the mailbox back or use everyones favorite Exmerge to get the email out. I think it depends on the environment that you have.
