It blows my mind that Microsoft is dipping its toes into the waters of telephony and doesn't even remember to include MWI in their product. That's not the way to inspire trust that they understand telephony, IMO.
There is no standard for MWI on traditional PBXs, that's why it doesn't work. Every vendor has a different method for triggering the light, and not all of that is available to external solutions like Exchange.
That's why OCS has a missed call count. That's why many phones say "voicemail waiting" or something like that, in their display.
A solution is coming, but not in 2007, AFAIK.

Novistech does the MWI feature of Unity not function at all or is it delayed (by an hour or so)? Has it ever worked in this environment? What version of Exchange is the Unity integrated with (assuming Exchange 2007 but that may not be the case)?
