Excel charting problem, please help

Hi,
I need to create a bar chart that shows YTD totals for 10 different codes, AND for each different code bar also show the total number that have more than one code (is this a stacked chart??)
I have 2 sets of data, YTD totals by code and YTD totals by code that contain more than one code.
My problem is I cannot seem to display the chart as is needed, see below:
code bar 1.(# with more than 1 code)...(actual total for code 1)>
code bar 2.(# with more than 1 code)...(actual total for code 2)>
code bar 3.(# with more than 1 code)...(actual total for code 3)>
code bar 4.(# with more than 1 code)...(actual total for code 4)>
etc.

My 2 sets of data are set out seperately (non contiguous) as follows :
month, code 1, code 2, code 3, code 4, etc
April,
May,
June,
July,
Aug,
Sep,
Oct,
Nov,
Dec,
Jan,
Feb,
March,
YTD total,

Please can anyone offer any pointers to what I'm doing wrong. I am an Excel charting novice.
Any help or advice would be much appreciated.

Please post some representative sample data, clearly annotated, that can be copied and pasted into a sheet.

Please explain what you expect to see with regard to your posted data.
