esign robot not uploading pdf to workflow

I have a workflow that utilizes the esign robot step, and everything seems to work except the pdf does not get uploaded back to my workflow attachment. I have verified that adlib does convert the document to pdf, and places it in the output directory. Then, the pdf dissappears from the output directory after a few minutes, so it seems to get lost in space.

I didnt experience this issue in our dev (2 server environment), however, the issue started happening when i migrated the workflow to our QA (3 server environment). I'm guessing it has something to do w/ cluster configuration, but everything seems to match up w/ the install guide. I will add that I have successfully got it to work in our QA environment, but its few and far between.

Any of this sound familiar to anyone?
 
Yes since it is a agent driven thing I would check what the loader line looks in your opentext.ini
also put debug=2 and see if some other agents are causing a hiccup.
also esign needs admin servers on all boxes to run and working as it uses that to do its "automagic"
I would also do a comaprison in agentschedule table for a agentid that you know works and the esign agent.
