ES 470 - 48 pwr switch error

Hi all.

We have a problem with a couple of ES 470 -48 pwr switches. They sometimes display the following error on startup:

Test 212 NSXs Internal Loopback - FAILED
NSXs ILB NSX: 16. Rx Failed: Mode=5 Octets Wrong: Sb=1108. Is=0.

When this happens the boot process stops and the "agent code" menu is displayed - one option allows you to "run" the agent code. However, if you choose this option the switch would boot but the status light would not come on.

What could be causing this error? We've tried downloading different firmware but the problem persists. It's intermittent so that you wouldn't get the error all the time. Plus it is affecting more than 1 48-port switch.

Sounds like a hardware issue - you choose to start the agent code does the switch pass traffic? I'm wondering if one group of ports has failed, but don't recall how the ports are divided up on these models.
 
The switch does pass traffic once you start the agent code. I'm wondering if it has anything to do with the cascade module. But since with the 470's they are built-in, there's no way to remove them to test that theory.
 
In certain releases of code I believe you can run a cascade module loopback test. You connect the cascade up and down ports together via a cascade cable then run the diags from the boot menu (press control-i on bootup). What I'm not remembering is if that works on 460/470s, or if it only works on 55XX switches.
