Errors after upgrade to 5.4

Ever since we upgraded to v5.4 we have been receiving numerous errors such as the following:
NetWorker media: (alert) Jukebox `lxb1' failed Thu 07:04:58 dlt7000 tape /dev/rmt/lxb1_dlt3 is writing

Any ideas?
