Error with VXDISKADM Option '5'

Please I removed a faulty disk using vxdiskadm option 4, but could not replace it the option 5. I am getting the following error.

VxVM ERROR V-5-2-1985 No devices are available as replacements for <disk_name>

Please, any idea on the way forward?

Was the disk genuinely faulty? Have you replaced it? What is its state in vxdisk list output? You will probably find two entries related to that disk, one where the original disk was listed, and another at the bottom saying "removed: was cNtNdN" or similar; please post both of them.
