ICA uses DNS to resolve names, so first place I'd suspect is the client DNS settings.
Try also using the IP address of the MetaFrame server in the Server Location field in the client.
If this is happening on a LAN, then check the DNS settings on the DNS server - I've seen many instances where the admin for the DNS server was sure it was all configured correctly, but the Citrix server had somehow not been included in the Reverse lookup list!
