error code 42 with mksysb command 1

When i try to get a sysbackup with "smitty mksysb" i've got this message : Creating tape boot image.fshop_make: 0506-269 Internal error, bread/bwrite failed .
Filesystem Helper: 0506-526 Implementation-specific error, code = (142)

0301-155 bosboot: mkfs failed for /tmp/Bootram.fs.
0512-016 mksysb: Attempt to create a bootable tape failed:
bosboot -d /dev/rmt0.1 -a failed with return code 42.

Someone could help me ?

what version of the operating system are you running ?

Also , have you checked for basics , like any errors in error report , correct tape settings , dirty tape , bad tape
