Error Code 40877 when trying to add supervisor

I am getting an error code: 40877 'The system is unable to complete the desired action' when I try to add a new user (supervisor) into my Symposium. Has anyone seen this error? Thank you.

On what system? SCCS 4.x, SECC, SWC 4.x CC6?

Do you use special logins?
e.g any login name should not interfere with some SQL statements, test is not possible for example.
