Error adding logins to CM 4.0

While trying to add a login I consistently get:

Error modifying user parameters, system call failed.
Password: Your account is locked. Maximum amount of failed attempts was reached.
Error modifying password aging parameters.

Yet I can login to both the web interface and via ASA with no issue. I get similar errors while trying to do anything with logins via the web interface, yet all other functioans seem to be fine.
