This happens when someone has swapped phones with the wrong type ( ie... a M2616 in place of a M2008) or a bad connection ( jack, cable, punch down...ect.)The system is seeing in put from the set that it thinks is coming from a key that it doesn't have. Check the set, connections and ect.
If its not working, get a bigger hammer!

Likely related to an add-on module - maybe you swapped it with a non-module TN like Splic3d said? Or the add-on module may not be setup correctly ...
