Email survey in "HTML FORM" style

Hello Tek Tippers,

I have a great data base for my street mag going on.

My problem with my email newsletter, i just want to be able to include a few qestions in a form style with check boxes, text feilds and menus so they can submit back to me.

I don't realy care if they come back to a CGI or just as an email would be fine

You don't know how to create a form in HTML?
