email form to 3rd party

As part of my libray skills instruction, I have a form the kids need to fill out and print. I would like to give them the option of sending the completed form to their individual email accounts.

Is there such a script, etc.?

Hope theres's someone out there who knows. I've been searching without success.

I contacted my tech guy at school. I think the heat has made him grumpy, because his answer to your query was:
"I use asp scripts to send forms to email addresses"

I emailed him back for server software details. He's upgrading our system over the long 4th weekend.
