Electronic Hire/Fire Form?

Greetings.

I was not too sure where to post this.
I am looking for a pre-defined form that can be emailed back and forth from Managers to me with all the info for a new hire or an employee that has been terminated. This form would have all info needed to setup the new user with email, passwords, etc. Also a means to check-off what needs to be deleted/removed when a person is let-go.

Does anyone out there use anything like this currently?

Why not create fillable PDF forms? That way you decide what goes on the forms. Just be sure that you set the permissions of the PDFs so that users with Reader can save the document with the filled out form-fields.

Or, if you're using Outlook/Exchange; you can create custom mail forms that are then emailed to you.
