Efficient way to design a Data Entry Form with 100 questions

Hello all,

Need your expert on efficient way to design a form.
I have constructed an interview questionnaire with 100 questions. The table structure is:

Interviewee
Interviewer
Supervisor
Department
Date
Question_number (range from 1 to 100)
Score (3 choices, either 1, zero, or blank)

The first five fields will be repeated 100 times per one questionnaire.

My questions are:

1. What is the most efficient way to create a data entry form? I would like the person who perform an input, can finish one questionnaire as soon as possible.

2. Should I just create 2 fields in the form? It will work for the first 100 (to repeat the same information.) Record # 101 might be different because of different interviewer and different supervisor.

3. I need to create the table this way because I can create a report based on specific question number.

Any advices from the experts? I use the Wizard to create the form. I use ACCESS 2000, not 97. Thanks.

I am slightly confused by what your have written. Surely it would be far more efficient to simply have a table containing the fields for 'Interviewee', 'Interviewer', 'Supervisor', 'Department', & 'Date' with a primary ID field. Then create a one-to-many relationship a table containing a the question no (which can be the primary key for the table), & the answer to each question. That way the 'slow' data need only be entered once per questionaire. You could normalise this further, but that depends on how many surveys each interviewer completed. As for a data entry form, the user would then enter the main details for each survey, & then could then simply tab through the 100questions reasonably quickly......
