Need your expert on efficient way to design a form.
I have constructed an interview questionnaire with 100 questions. The table structure is:
Interviewee
Interviewer
Supervisor
Department
Date
Question_number (range from 1 to 100)
Score (3 choices, either 1, zero, or blank)
The first five fields will be repeated 100 times per one questionnaire.
My questions are:
1. What is the most efficient way to create a data entry form? I would like the person who perform an input, can finish one questionnaire as soon as possible.
2. Should I just create 2 fields in the form? It will work for the first 100 (to repeat the same information.) Record # 101 might be different because of different interviewer and different supervisor.
3. I need to create the table this way because I can create a report based on specific question number.
Any advices from the experts? I use the Wizard to create the form. I use ACCESS 2000, not 97. Thanks.
