I have a G3 Version 12 with licenses for 5 EC500 users. When I program ec500 on a key and set up the cell phone number the system should call, everything looks good, except it doesn't ring the cell phone (or any telephone number I put on the off-pbx-telephoen list). Is there something else that has to be set up on the system to use EC500?

outgoing calls must route to the cell numbers over isdn trunk facilities.

I would post your "display off-pbx-telephone station mapping" you might have it misconfigured. Either that or run an trace on the extension to see the call flow.
 
If you status the station, does it say:

EC500 Status: enabled Off-PBX Service State: in-service/idle

If you have EC500 activated on extension 1234, what happens when you put a trace on 1234 and watch the output as you call it?
