Only on the sim questions, but most sims are testing basic configs - you should know them like the back of your hand from practice practice and more practice before you take the CCNA!
Have seen many students who take the exam before they are ready and get frustrated by the sims and end up taking way too mucg time on them.
For sims - I would suggest that you PRACTICE, PRACTICE and then do some more PRACTICE before you take the CCNA exam. You will not have the time to finish the exam if you must use a lot of help! Most of the sims are for basic configs that you should be able to do in you sleep - if you are not at that stage - you will have trouble on the exam.
Read the other post where the poster could not finish the exam with the time he spent on the 3 sims - he was a prior CCNA and is working on CCNP. You do not have time to figure it out on the exam. Help is ok if you get stuck on 1 little part!
"Do they have a lot of the questions that expect you to know a command string by heart?" - YES and NO you might have to "drag and drop" a long command into the right order or you might have several multi-choice where all 4 answers look the same or have the same keyword in it such as which "show cdp" command to use - show cdp, show cdp neighbor, show cdp entry, show cdp interface etc.
Lots of ways they can test if you know the IOS commands! The key here is to learn them - with practice it gets easier
