e911 not working, denial event

Must be an ARS issue of some sort. Trying to dial 9+911 and getting the following denial event when doing a trace...

denial event 1166: Unassigned number D1=0x8a61 D2=0x201

Caller gets turkey-tone. Any ideas? Thanks.
