The two highlighted columns above are database columns that were added with Custom Text 5.1 Service Update (see enclosed document). 5.1 did not install properly and thus these columns do not exist in the database. If the site is installed with 5.1 from RDP session, that may not have been logged in correctly. To install or upgrade any Software on Call pilot console mode should be used or things will not work properly. If the MAS Trace window is not visible when logged in through RDP, then you are not logged in correctly.
SOLUTION
1. Uninstall CP0501SU001S.
2. Reboot server.
3. Reinstall CP0501SU001S.
The SMTP info hasn't changed has it - IP and/or FQDN either on the CP or the network folks changed it? Email notification requires SMTP info added to CP. CP uses the SMTP server as a relay agent, without it, messages stop working. You can run a SMTP trace on the CP server to see if it's hitting and leaving the SMTP server. You can search threads on here about SMTP traces to get you going.
