DVC Part No. for NAM II 2.0

I need the part number Norstar Digital Voice Card for NAMII 2.0. This is going to be the 2nd DVC for the system. Please correct me if I'm wrong, I'm installing this in slot number 5. Can someone also tell me the dip switch setting for this card?

Don't know the part number of the DVC, but the 3rd switch on the second DVC should be up. All other jumpers will be the same as the first DVC.
