duplicate outlook email being sent

Hi I have two clients who randomly experience the same problem. Which on occasion emails that have been sent the once arrive at there destination twice. Same email same sent time. (Both on outlook 2003 and 2007)
Mailbox diagnostics passes. Add on's have been disabled. And rules have been checked and disabled however the problem persists.

Any help to resolve this would be appreciated.
