Duplicate Clock-Ins

I was presented with an issue today that I'm seeking some help with. There is a certain server who gets duplicate clock-ins whenever she works. When they try to edit her times they receive an error that says it's corrupted and they are unable to change her hours. Can anyone shed light on this?

If you are using Aloha, I've seen on occasion where an employee has issues due to having a duplicate record in the database. I usually just deleted one of them, and the issues went away.
