I have recently been asked by the boss to setup voicemail in the system. I go to the 192.168.1.250 to login to the phone manager software. It asks for the phone extension (3 digit) and PIN (4 digit). Is there a default extension and PIN? If that doesn't work, is there a way to reset the extension and PIN number without wiping all the other settings out? Thank you.

Are you attempting to log into the phone system with DSX System Administrator?
If so, the published default log in and password is NECDSX + 632379.
Phone manager, or IP phone manager is used to log into an IP phone, not the system.
Voicemail boxes can be set up from a phone with VM system admin rights. Usually extension 300 or 301 depending on your software version. Press the VM button, dial 72 after the mailbox answers, then follow the prompts.

Ah, ok. Thanks. I can connect to the 192.168.1.250 through my Firefox browser on my PC. However, I cannot connect to the IP when I try it on the System Administrator software. Is there a specific TCP port one must use? I tried it with no password and the 632379. It just says Ethernet Error -Unable to connect.

Try resetting the system. Also make sure your sys admin program is the proper version. System software 2 uses sys admin2, 3 uses 3. If you used it to connect in the past then it's probably the right version. Back to trying a reset. You might also try another ether cable and verify your PC is on the same subnet as the system.
