DSS on 5420

I have a customer with a 406V2 at 4.1(12). The receptionist has a DSS module. She is complaining that the direct to voicemail is not working (transfer->#->ext number or User button on DSS). She also says that sometimes she will press the user button on the DSS and it will not work. Also says sometimes when she tries a VM Xfer the screen on the phone will go out and come back? Any help is much appreciated.
 
If she just press DSS button for user does it ring the person>? Is the user programed on DSS?

Can anyone else use Direct VM..

She is the only one answering phones/directing callers to voicemail. All of the users are programmed on the DSS. Sometimes when she just presses the user button on the DSS it works and sometimes it does not. I will have someone else try to send someone to voicemail to see if they can do it.
 
Does she presses Transfer or disconnects?

transfer-> # + ext number or User button on DSS -> TRANSFER.

I don't think she is completing this with a transfer at the end. I believe she just hangs up.
