DSM 32 on BCM400

I'm trying to swap out a DSM16 with a DSM32 on a Nortel BCM400 4.0. I've set the dip switches accordingly, installed the DSM32 and it lights up green. I try to plug a phone into the new port, and it blinks nonstop. What am I doing wrong here? I"ve done this plenty of times with no issues until now.
 
Yes. It doesn't give me the option to choose DSM16 or DSM32, I only get to choose DSM. I believe it's the 450's that let you get more specific.
 
If this is a BCM450 and the DSM is in the main cabinet you don't have to set the DIP switches, leave them all off. The DIP switches only need to be set if the DSM is going in an expansion cabinet.

 
When I connect a phone to the other cable, the phone does the initialization blink, and gives time and date, but never boots up. And yes, this is a 400, not a 450
 
Have you set the first dip switch to on.
Then set the others to the bus you are using
Eg 1xxxx1 bus 3
eg 1xxx1x bus 4
 

Might interfere with a trunk module,how many trunk/type modules do you have in that bcm?
