DS30A and DS16A Modules

A customer has an IPOV2 cabinet with 2 expansion modules for Norstar phones, DS30A and DS16A. I have had a problem with the DS16 losing signal with the IPO and not coming back up. I was finally able to get it back up by putting it in a different expansion port, but it was not an easy process. Our distributor has an open case with Avaya, but no once can explain why this happens or if there is an easier solution. Any thoughts?
 
i had the same exact issue today ds30 for nortel phones lost signal, took a few well timed reboots to get it back up

this was a brand new install 8.0.44
 
I had it happen last week just before the cutover, and I got it back. It happened again at the end of that day and I was never able to get it back. I even had another module overnighted, and that one did not work either. I did get a 30 and a 16 to work on my test system today. It was tough to get them working, but it did eventually happen. What puzzles me is the fact that Avaya is aware of the issue but has no explanation or real fix for it.
