DS16 module keeps rebooting, not seen by main unit

In a customer configuration 406V2, DS16, Phone8 and another DS16, the last DS16 with only 4 phones connected suddenly keeps cycling trough red, green, dimmed red and red again.
Software version 4.07.
In system status the unit is not visible. I rebooted the whole system no result.
I removed the DS16 and connected it to a IPO403 with 3.2.69, no DS16 visible when trying the upgrade wizzard. I use the DTE i/f on the DS16, went trough AT-X3 I saw the poll sent, load and completed. after the version load still the same problem. I changed power supply, used another blue connectioncable, no result.
Does anybody have a suggestion to try?

Sound like a fried flash chip.
Can you post a DTE trace ?

The cycling through red,green etc usually indicates the module has gone bad and is not recoverable, it will need replacing, we have seen this a lot with DS30's recently...good thing they are cheap...not, I personally beleive this is one of the reasons Avaya is reducing the warranty to 1 year, this and 500 power supplies they want us to foot the bill for the shoddy kit from now on
