DS1 Amphenol -> CSU Cabling 1

I am having a hell of a time getting my very first pri up and running, and I think it may be my wiring.

I have the amphenol cable going to a T568B Connector, then a straight through to a t1 RJ45->DB25 DTE adapter to a DB25->V35 adapter then into the CSU (Adtran CSU)

All connectors were composed from the designs found here:

I used that page and the data on page 457 of the option 11c dti/pri admin guide.

I cannot get the card out of red alarm. Additionally, I can not get the transmit light to flash or come on at all on the csu.

I know the T1 is functional, we tested it on a the CSU attached to a cisco router.

I know there may be many things that I am doing wrong.. I am just looking for some ideas that I haven't thought of yet.

Unfortunately I'm not sharp enough to answer to the specs you provided. However, I do have some info that may help. On a DB15 connector Pins 1 & 9 are transmit. Pins 3 & 11 are receive. You can make a loop-back device using a matching connector and wiring the transmit pair to the receive pair. So let's say you are connecting to the network using a Male RJ-45 type connector. Obtain a female RJ-45 jack and at the jack connectors where you would normally connect a cable wire the transmit to the receive. This will make a loop-back plug that you can use for testing. Unplug your cable from the network and plug it into the loop-back device. If your PBX T1 comes up then the wiring might be reversed. Swap the transmit pair with the receive pair in your connections. If the T1 in the PBX is still red then you have wiring problems.
 
Your advice is gold. I have traced it back to the RJ45->DB25 adapter i have wired. I thought that everything would be standard from that point out, but I guess not.
