DS 16 V1 Not working

Hello I have an IPO 403 running 3.1.65. I also have a DS 16 attached. It has been upgraded to 5.1.65 which was available with 3.1.65 (manager). It was working but now I only getting a ticking sound on all the ports. Manager can see it and the LED's go on when a phone is attached (6408D+) but nothing but ticking. I tried to upgrade again to the same version but it fails. Any ideas? Can you erase anything through the DTE port. Any ideas are much appreciated.
