Display reading "not in service"

I have a customer who has an 0x32 with a flash 4 and about 30 phones. When you call random extensions or try to transfer a caller to random extensions the display reads "not in service" is there anything to test to see what is causing this problem? Or is the system just starting to crash?
 
What are you calling RANDOM extensions, you will get that display if the extensions don't exsist or the phone is unpluged OLD ROLMEN WORKING ON NORTELS
 
no it just happens consistently on random ext that are working. It gives you from time to time the same response as if not ext exists
 
Have you seen it yourself and been able to duplicate it or are you taking the customers word.
The reason I ask is it sounds like user error OLD ROLMEN WORKING ON NORTELS
