Display only top 5 dates in a report

I have a report that shows all procedure totals for each time a surgeon completes a procedure grouped by procedure type.

Is there anyway to write a formula to show only the last 5 procedure dates?

Example: date of surgery (all are for the same type of procedure)
6/13/2011
6/13/2011
5/23/2011
5/23/2011
5/2/2011
3/14/2011
3/7/2011
4/5/2010
3/1/2010

I only want the report to show the first 5 dates (6/13/2011 – 5/2/2011).

These dates change based on the surgeon and procedure
Any help would be appreciated.

You should be able to insert a summary on the date (maximum) at the procedure type group level and then go to report->group sort and select top5->choose the summary and select ascending or descending (not sure which--just test).
