Disks 3 and 4 does not appear in lsdev - are they scsi disks ?
Please post the output of :

lsdev -Cc disk

If yes - they are both not available now.

They both seem to fail around the same time - it's to do with the way they are connected.

the output of lsdev -Cc disk :
hdisk0 Available 40-61-00-0,0 16 Bit LVD SCSI Disk Drive
hdisk1 Available 40-61-00-1,0 16 Bit LVD SCSI Disk Drive
hdisk2 Available 31-08-3L-0,0 SYMMETRIX VCM Base
hdisk3 Available 31-08-3L-1,15 SYMMETRIX RAID-1(local)
hdisk4 Available 31-08-3L-2,0 SYMMETRIX RAID-1(local)

So,You have a RAID - hdisk3 and 4.

Clearly you are having problems with your RAID controller.
I would run some diag/checks/ and view it's logs (if available).

you will need to troubleshoot this from the EMC frame, AIX can't do anything worthwhile to a Symmetrix disk. actually EMC Corp should already know about this, assuming you have support. are you running powerpath?

btw, DISK_ERR4 is a media error, sometimes associated with a flaky SCSI connection whereupon you would see SCSI_ERR10 in the error log. this does not apply to you unless generated by an IBM disk on a SCSI channel... your errors are on EMC disks.
