maybe disk error?
or you should probably start logging queries run on your mysql server and look there if somebody is not issueing DROP commands

good suggestions,.
have checked for disk errors and turned on logging (after the fact sadly).
may have found the problem - in re-reviewing users accounts and permissions, we found one that had way too much privilege and way too little knowledge. this hole has been plugged.
will monitor and keep an eye on the logs.
