Disappearing merge fields

I'm using InDesign CS with the PM plug-in on Win2K. I'm merging data from a .csv into a multi-page booklet. Occasionally, I notice 1 or 2 merge fields have disappeared, and are replaced with text that was previously merged into that location. I can't seem to track down the cause. Has anyone else had this happen to them, or know where I might find more info?
