digital to ip

I am trying to find out the steps I need to take to move an phone from a digital to an ip phone on a bcm 400. I want to keep the same extension etc..

Connect the IP phone, assign an extension to it, renumber that extension to the extension of the digital set, assign lines, line pool access, CFNA, etc.
