IPO500 with the older white digital station modules
I was doing some wire maintainence on the rack.
Prior to unplugging, phone was solid...

Plugged cable back into the IPO module. No connection.
Fast forward. Ended up checking the run with the DTX back to desk port.
Checks out.

Take the phone and cable from desk and plug direct into IPO port.
Phone works

Take it back to the desk. Plug it into the port off the wall, no dice.
From the port from the desk, back to the patch, it checks out fine.
The cable between patch port and panel, I even plugged the phone with this cross connect, and it works fine.

So in theory, all the stars are aligned but I just cannot get this to work when back at the desk.

What gives.

The system has no idea how the phone is connected to act any differently and your tests prove its the cabling, if your tester doesn't show anything wrong then it isn't testing the factor that is causing the failure :)

Also bear in mind the test heads are a tighter fit and tend to insert further into the wall socket than a standard patch lead, I have seen a tester pass but standard patch cable/lead fail many times :)

I once had to drive 320 KM only because the patch wires weren't put in the wall socket thoroughly, cheap cables turned out to be expensive after all.
