DID Translation

Hey guys we have a 2000 ips and we just had to new did's ported over from the phone company. I need to create translation but I have not done this before. This is what I have so far. Can someone point me in the right direction? The last 4 digits of each DID are 6100 and 6150. Is 040 the Table code and why is it transferred to trunk d14. Do i need to just change d14 to an extension number? Thanks

Heres what I came up with. Right now the phone company has the numbers forwarded into some other did's I temporarily set up and once i set this up I need to tell them to remove the forward. Will this then work for me?

Make sure that your DID route is set to translate digits.
